    Description:

    The way in which clomiphene works as a fertility drug is by acting on the ovaries and on the pituitary gland and hypothalamus to raise levels of certain hormones. These are follicle-stimulating hormone (FSH) and luteinizing hormone (LH). When clomiphene is taken at specific times of the month, the increased levels of these hormones may help produce normal ovulation, thus elevating chances of pregnancy occurring. It tends not be effective when estrogen hormones don’t fall with normal ranges. Sometimes women have a higher than normal FSH level to begin with, and in this case the drug may not be particularly effective.

    Application:

    For women:

    1) Stimulate ovulation in a woman who does not ovulate or who ovulates irregularly.

    2) Perform a clomiphene challenge test, which is sometimes used to evaluate a woman’s ovulation and egg quality (ovarian reserve). When given early in a woman’s menstrual cycle for 5 days, clomiphene elevates a woman’s follicle-stimulating hormone (FSH) level. On the next day, an FSH blood level that has dropped back to normal is a sign of a normal ovarian reserve and ovulation. An

    elevated FSH is a sign of low ovarian reserve. Women who have a diminished ovarian reserve can use donor eggs, which greatly improves their chances of giving birth to a healthy child.

    3) Stimulate the development of multiple eggs for use with assisted reproductive technology, such as in vitro fertilization (IVF) or gamete intrafallopian transfer (GIFT).

    4) Regulate ovulation in a woman who ovulates irregularly and/or is going to have intrauterine insemination or artificial insemination.

    For men:

    Clomiphene may be used to treat low sperm counts (oligospermia).

    Dosage:

    Usual Adult Dose for Ovulation Induction

    50 mg orally once a day for 5 days. Therapy should be initiated on or near the 5th day of the menstrual cycle, but may be started at any time in patients without recent uterine bleeding.

    If ovulation occurs and pregnancy is not achieved, up to 2 additional courses of clomiphene 50 mg orally once a day for 5 days may be administered. Each subsequent course may be started as early as 30 days after the previous course and after pregnancy has been excluded.

    Most patients ovulate following the first course of therapy. However, if the patient fails to ovulate, a second course of 100 mg/day for 5 days may be given as early as 30 days following the initial course. A third course of 100 mg/day for 5 days may be given after 30 days, if necessary.

    Treatments beyond three cycles of clomiphene, dosages greater than 100 mg once a day, and/or course durations beyond 5 days are not recommended by the manufacturer. However, successful pregnancies and term deliveries have been reported in women receiving up to 200 mg/day for 5 days, or extended 10-day course of therapy, or consecutive cycles of treatment beyond the 3 recommended by the manufacturer.
